Direct Primary Care Membership
Oaks Health Associates PC is a direct primary care / cash-pay practice. This means we do not bill or accept insurance. Instead, you pay a predictable monthly membership fee or transparent visit fee, giving you easier access to your provider and fewer billing surprises.
No Insurance Billing: Our practice no longer takes insurance. Patients may still use their insurance for outside services such as labs, imaging, or specialist visits as allowed by their plan, but all care provided by Oaks Health is billed directly to you.

Individual Adult Membership
$125/month
● Comprehensive primary care for one adult● Annual wellness exam + routine follow-ups● Secure messaging with your provider● Discounted cash pricing for labs and procedures (where available)
Couple Membership
$200/month
● Comprehensive primary care for two adults● Annual wellness exam + routine follow-ups● Secure messaging with your provider● Discounted cash pricing for labs and procedures (where available)
Child / Young Adult Membership
$65/ month
● Primary care for one child, adolescent, or college student● Wellness visits and acute sick visits● Care coordination as needed
Family Membership
$275/month
● Affordable membership for households● Bundled primary care for multiple family members● Priority scheduling and easier communication
Enrollment Fee
$35 one time
● One time enrollment fee is required for all memberships
*Memberships do not include medications, outside labs, imaging, or specialist services. We will work with you to help make these as cost-effective as possible.
Pay-Per-Visit (Cash-Pay) Options
For patients who prefer not to enroll in a membership, we offer a limited number of cash-pay visits at transparent rates. Membership patients receive the best value and access.
